To Enroll You Child You Need:

• Verification of age (with one of the following):
  
  1. Birth Certificate
  2. Insurance policy
  3. Passport
  4. School record
  5. Certificate of baptism, accompanied by parent's affidavit
  6. Bona fide Bible record, accompanied by parent's affidavit
  7. Affidavit of age sworn to by parent, accompanied by a medical practitioner's statement

• Proof of Valid Florida DH 680 Immunization Form.
  The form can be obtained at the Department of Health:

MMR (Mumps, Measles-Rubella)

DPT (Diphtheria, Whooping Cough and Tetanus)

OPV (Polio and Hepatitis)

Kindergarten also Requires

• 5 DTP Series
• 3 Hepatitis B
• 4 Polio
• 2 (MMR) Measles, Mumps, Rubella
• 1 Varicella (Chicken Pox)

Pre-Kindergarten Requires

• (HIB) H. Influenza Type B

Pre-Kindergarten to 5th Grade Requires

• 1 Varicella (Chicken Pox)

• Proof of Residence, which can be any one of the following:
  • Current Homestead Exemption Card
  • Lease/Rental Agreement (if you have lived in Florida more than one year)
  • Verification of address (if you have lived in Florida more then one year)
